ORDER:
The Commissioner of Health and Family
Welfare, AP, Hyderabad in his
letter read above has reported that
Immunization is one of the most well-known
and effective methods of preventing childhood
diseases. With the
implementation of the Universal
Immunization Programme (UIP) by the
Government of India, significant
achievements have been made in preventing
and controlling vaccine-preventable
diseases (VPDs). Immunization safety has a
wide spectrum ranging from vaccine
manufacturing & regulation, vaccine safety
& quality, safe injections &
waste disposal and Adverse Events Following
Immunization (Adverse Events Following
Immunization) surveillance. An
effective immunization safety
surveillance system must be able to detect and
differentiate the types of Adverse
Events Following Immunization in order to
prevent their occurrence and/or reduce
their impact. The objectives of
strengthening Adverse Events Following
Immunization reporting at all levels,
ensuring maintenance of national policy
and standards, and ensure prompt and
thorough investigation of serious
Adverse Events Following Immunization and
periodic review of Adverse Events
Following Immunization for trends of Non
serious Adverse Events Following
Immunizations reported through HMIS
Adverse Events Following Immunization
committees are to be established at
district level. The District Adverse
Events Following Immunization committees
were constituted earlier in the combined
state of AP. They are to be
reconstituted as per the revised
guidelines which came into existence in 2010
and were communicated by Government of
India, since the state of AP is
bifurcated and in view of the proposed
launch of pentavalent vaccine and
subsequent routine immunization he has
requested the Govt., to issue necessary
orders duly constituting the District
Adverse Events Following Immunization
(AEFI) Committees in the state of AP as
per the guidelines of Ministry of Health
and Family Welfare, Government of India.
The following are the District wise
allotment of the Medical Colleges:

of the matter in detailed and hereby
constitute the district Adverse Events
Following Immunization
(AEFI) committee with the following
members:
3. The Terms of Reference for District
Adverse Events Following Immunization
(AEFI) Committee are as follow.
To analyse the FIR
(First Information Report) and plan for investigation of
AEFI as a team
To provide appropriate
information to the drug authority on the important
aspects of temporary suspension of the
implicated batch of
vaccine/logistics.
To prepare DIR
(Detailed Information Report) based on the findings of the
investigation of he AEFI.
To outline the further
course of action on the current AEFI.
To analyse and review
the quarterly AEFI data for any programmatic
errors and remedial measures for the
same including non-serious AEFI
cases reported in HMIS.
To participate in the
state/national AEFI committee meeting, if required for
causality assessment.
To support he
spokesperson for media communication.
Where needed facilitate
in incorporating the message of reporting AEFI
from all sectors (including private
sector).
To monitor the timely
submission of completed investigation forms (FIR,
PIR and DIR) along with supporting
documents/medical records.
To communicate and
share the conclusions and results of investigations
with health workers and the community
where warranted.
Any other
responsibility in context to vaccine safety.
4. The Commissioner of Health and Family
Welfare, AP, Hyderabad shall
take further necessary action in the
matter.
